In the early stages of planning the Third International Conference in System Science in Health Care, the steering committee members, most of whom had participated in the first conference in Paris (1976) and the second in Montreal (1980), made some basic decisions about organization of subject matter. The earlier meetings had been very successful in bringing together specialists from the health professions and the traditional sciences. In addition to physicians and nurses, these were representatives of the disciplines of the behavioral sciences, system theory, economics, engineering, and the emergency fields of management science and informatics -all concerned with the development of health resources in a broad system context. The reported research and experience of the many disciplines represented had dealt with one or more of three concerns: 1) a major health problem, such as cardiovascular disease, or an important popUlation at risk, such as the elderly or children or workers; 2) some generic aspect of organization and decision making, including trial and evaluation ofinnovative health strategies; and 3) the methodology of research and analysis in system of health service. The challenge to the conference organizers lay in the eliciting and arranging of experiences in such a way that the health services could be seen as purposeful,living, evolving systems.

The roles of apoptotic genes, MYCN gene, MDM2, and SNP309, P13K inhibitors, alternative splicing and microRNAs, activated leukocyte cell adhesion molecule and inhibition by alu-like RNA in neuroblastoma are discussed in detail. The book explores the molecular genetics, diagnosis, prognosis and therapy of the atypical teratoid/rhabdoid tumor (AT/RT). Among the most common malignant neoplasms in children, AT/RT exhibits similarities with other CNS tumors, which can lead to misclassification, as pointed out in the book. The contributors discuss diagnosis of AT/RT type using imaging technology, and describe new strategies, including intensive multimodal therapy and high dose chemotherapy with autologous stem cell transplantation that have shown improved outcomes. Coverage of therapies includes total resection followed by aggressive chemotherapy and radiation. Discussion includes diagnosis and treatment of other pediatric tumors including adrenocortical tumors, supratentorial primitive neuroectodermal tumors, giant midline tumors, gastrointestinal stromal tumors, ependymomas and intramedullary cavernoma. There are over 180 million people with chronic HCV infection worldwide with between 2.7 and 3.9 million in the United States. Hepatitis C most significantly affects Asia and Africa, with rates up to 15% in countries such as Egypt and up to 30% in certain regions such as Punjab, Pakistan. Hepatitis C places a significant burden on the public health infrastructure, as it remains the leading cause of chronic liver disease, accounting for 50-75% of primary liver cancers and is responsible for 30% of all liver transplantations. It is estimated to have cost the United States $5.5 billion in 1997, comparable to the national cost of asthma, $5.8 billion in 1994.This number is only expected to grow as the current HCV population ages, increasing overall rates of compensated cirrhosis/end-stage liver disease. The evolution of directly acting anti-virals has ushered in a new era for chronic hepatitis C. Ongoing drug development strategy has involved targeting several replication steps of the virus and the hope is to see all oral therapies by late 2014 or early 2015. Thus we are at an exciting cross roads with regard to new information and challenges with HCV: rising disease burden with associated high costs, the challenges globally and in multiple patient populations, and the impending availability of effective and well tolerated treatments. Therefore there is a need to commission an exclusive issue of GCNA for HCV.
