Lacuna, defined in the Oxford Dictionary as a Hiatus, blank, missing portion, empty part, seemed to be the picture of the world to J, whom on the death of his exhausted mother, becomes the sole carer of his aged father who is in an advanced state of Alzheimers, in a confused, agitated South London where mobile phones and iPods, both addictive and sedative, are background Muzak to more serious displays of road rage, street rage and drug activity. Encountering a Health Service whose professionals find it as difficult as him to reach down into the heart of this unseen, ungraspable disease he is forced to seek innovative ways of dealing with the daily problems. A chance meeting at his local pub provides him with an informal support group. Three men that he meets in the late evening several times a week become to him the Three Wise Men, affectionately abbreviated to TWM. ln the third year when his father takes a serious fall they enter into a world of medicine which is advanced and competent but where 'nursing', has become clerical rather than physical, and where 'care' has been marginalised by pseudo professionalism. Whilst grim duty never goes away what began as a five year retreat from Moscow becomes in part a rewarding internal Odyssey. A gradual retrospective view of his father's early imperceptible decline does bring a degree of understanding and the lifelong unbroken friendship between them is reconfirmed.

In beautifully crafted vignettes, physician and NPR commentator David Watts explores the world of modern-day medicine and reveals the emotional truths and practical realities at the heart of the doctor-patient relationship. Bedside Manners is an engaging, often surprising investigation into what happens when we sit down and talk openly about vital issues of health and mortality. Combining the grace and precision of a poet with the down-to-earth, compassionate manner of a doctor who deals with the problems of real people every day, Watts describes situations both odd and touching: the patient who stays awake during an endoscopy to ward off demons; the woman who recites poetry to get through a frightening treatment; the man who arrives at Watts’s office bearing Internet research on syndromes that have little to do with his own condition; and the seventy-four-year-old architect who faces a tough cancer diagnosis with dignity and courage. Readers will come away from these tales of difficult diagnoses, irreverent colleagues, brave survivors, and examining-room poseurs sharing Watts’s own sense of humbled astonishment. As he tells each story, Watts closes for the reader the protective distance many doctors employ, and touches all of us who have felt vulnerable in the position of patient. Refreshing, wry, and reassuring, Bedside Manners holds important lessons for both healers and those who seek their help. From the Hardcover edition.

Health systems need to set priorities fairly. In one way or another, part of this important task will fall to physicians. How do they make judgments about resource stewardship, and how should they do so? How can they make such decisions in a manner that is compatible with their clinical duties to patients? In this book, philosophers, bioethicists, physicians, lawyers and health policy experts make the case that priority setting and rationing contribute significantly to the possibility of affordable and fair healthcare and that clinicians play an indispensable role in that process. The book depicts the results of a survey of European physicians about their experiences with rationing and other cost containment strategies, and their perception of scarcity and fairness in their health care systems. Responding to and complementing these findings, commentators discuss why resource allocation and bedside rationing is necessary and justifiable. The book explores how bedside rationing relates to clinical judgments about medical necessity and medical indications, marginal benefits, weak evidence based medicine, off-label use. The book highlights how comparative studies of health care systems can advance more effective and fair bedside rationing through learning from one another. From a practical standpoint, the book offers a number of strategies for health care systems and clinicians to work in tandem to allocate and ration resources as fairly as possible: how to foster more attention to fairness when rationing at the bedside, how to avoid exacerbating health disparities when allocating resources, how to teach about bedside rationing to students, how to discuss rationing more explicitly in the public arena and in the doctor's office.
